Please Note: this catchment map is for initial guidance only. If you live close to the catchment boundary or are unsure then you MUST ring the Reception team on 01278 722077. They will double check and confirm if your postcode and house number match the official Practice catchment list.
How to Register with the Practice a Step by Step Guide:
If you reside within the practice catchment area (see instructions above) you can register with the Practice. The preferred way is for patients to come into the surgery to complete the necessary paperwork as the forms must be signed before we can complete the registration. All new patient registrations are subject to Confirmation of the catchment area and the completion of a New Patient Form with forms of identification that includes your address.
PLEASE NOTE YOU MUST BRING TWO FORMS OF IDENTIFICATION ONE WITH A PHOTO AND THE OTHER WITH AN ADDRESS. WE CANNOT PROCESS YOUR REGISTRATION UNTIL ALL OF THE ABOVE STEPS HAVE BEEN COMPLETED.
Temporary Registration (Visitors)
If you are a visitor to the area, only staying for a short while, and need to be seen by a GP then you are able to register as a temporary patient with the Practice. Please go to the forms page and Complete the New Patient Temporary Registration Form (GMS3).

The doctors welcome new patients who live within our practice area. As it often takes sometime for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and are offered a consultation with the doctor.
Please book a 20 minute Self help Healthcheck appointment with our Health Care Assistant within 6 weeks of registering with us so that we can provide you with continuity of care.

We have a Patient Group who meet every 6-8 weeks or you can join the virtual Patient Group to whom we send newsletters, bulletins about surgery closure times for training and an annual survey in case you would like to fill one in. See our Patient Group page on this website.
