HOW TO REGISTER

Please Note: this catchment map is for initial guidance only. If you live close to the catchment boundary or are unsure then you MUST ring the Reception team on 01278 722077. They will  double check and confirm if your postcode and house number match the official Practice catchment list.

 

How to Register with the Practice a Step by Step Guide: 

Permanent Registration

If you reside within the practice catchment area (see instructions above) you can register with the Practice.  The preferred way is for patients to come into the surgery to complete the necessary paperwork as the forms must be signed before we can complete the registration.   

PLEASE NOTE: To permanently register with the practice you  will need to complete both the GMS1 form and a New Patient Questionnaire form in section 1 and 2 below. Print out and complete them but DO NOT POST to the practice! 

You must bring the 2 forms and two pieces of identification with you to the practice, one with a photograph e.g. driving licence/passport,  and the other with your current address e.g. utility bill.  We cannot process your registration until all these have been provided.


1     Print and Complete the  GMS1 Form by clicking the blue download bar. Only complete this if you have checked the catchment area and wish to join the practice as a Fully registered permanent patient.


2     Now Print and Complete the New Patient Questionnaire - Click here to download.
 

 On registration you will be allocated a named, accountable GP and it is hoped you will see this doctor for continuity of care. Patients have the right to express a preference to receive services from a particular doctor or professional group and the practice will endeavour to comply with any reasonable preference expressed. Nevertheless, the practice has the right to refuse such a request where it has reasonable grounds to do so.

 

Temporary Registration (Visitors)

If you are a visitor to the area, only staying for a short while, and need to be seen by a GP then you are able  to register as a temporary patient with the Practice.   Please go to the forms page and  Complete the New Patient Temporary Registration Form (GMS3).    CLICK on....  Forms Page

Bring this form with you to the practice.
 

Post Registration Health Check

The doctors welcome new patients who live within our practice area. As it often takes sometime for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and are offered a consultation with the doctor.

Please book a 20 minute Self help Healthcheck appointment with our Health Care Assistant within 6 weeks of registering with us so that we can provide you with continuity of care.

Please contact reception for further information including how to book appointments and request repeat prescriptions online.

Patient Participation Group

We have a Patient Group who meet every 6-8 weeks or you can join the virtual Patient Group to whom we send newsletters, bulletins about surgery closure times for training and an annual survey in case you would like to fill one in. See our Patient Group page on this website.
